Individual coverage: $250 maximum allowance per calendar year
Family Coverage: $500 maximum allowance per calendar year
Spa services available: Massage Therapy, European Facials, Reflexology, Far Infrared Sauna
and Eminence Organic Skin Care products.
2.Lifestyle Benefit Program ($0 copay) BCBS
Individual Coverage only: $250 maxiumum allowance per calendar year

3. Healthy Now NY ($8 or $ 10 co-pay) BCBS

FlexFit, FlexFit Select, HealthCare Allowance and Personal Best....use your eligble Master Card debit card for your massage services. Once your debit card is used up for the year you automatically switch over to the 20% discount program for your massage services.
